Calling all NHTSA Public Service Award Winners! If you received a NHTSA public service award in the past 30 years, please let us know. We want to include your name in the Awards Luncheon program and recognize you during the Awards Luncheon on Friday, June 15. Please send your name, organization, city and state, and the year you received your award to Judy Hammond at Judy.Hammond@dot.gov no later than Monday, April 16, 2012.

In order to process your registration, a check (in U.S. dollars, payable to Lifesavers Conference, Inc.), credit card number (Visa or MasterCard only – we do NOT accept American Express), or purchase order must accompany your registration form. Registration forms without an attached P.O. or payment will not be processed.
Your conference registration fee includes:
- Choice of over 80 workshops

Cancellation Policy (back to top)
- Lifesavers does not accept cancellations by phone. Cancellations must be mailed to Lifesavers Conference, or emailed to registrar@ptfassociates.com. You will receive a confirmation of your cancellation.
- Requests received by May 31, 2012 will be refunded less a $25 administration fee
- Refunds will be issued after the conference
- Requests made after May 31, 2012 or “no-shows” are not eligible for a refund

Scholarship Fund for Post-Secondary Students (back to top)

A limited number of registrations are available on a first-come basis at a reduced rate of $25 per person through a special scholarship fund for students interested in careers related to traffic safety, injury prevention, transportation public policy or urban planning. The early registration rate is $300; the scholarship provides a value of $275.
Eligibility:
- Students must not have reached their 30th birthday by June 17, 2012, and be enrolled at an accredited institution for the 2012/13 academic year on a full-time or part-time basis at the undergraduate or post-graduate level.
- A copy of your student I.D. must be sent with your application.
- Speakers, poster presenters, and exhibitors are not eligible.
- Recipients agree to stay at the Walt Disney World Swan and Dolphin.

By registering for the Lifesavers 2012 Conference, you agree and acknowledge that you are participating in Lifesavers Conference events and activities on your own free and intentional will. You acknowledge this freely and knowingly and that you are, as a result, able to participate in Lifesavers Conference events and hereby assume responsibility for your own well-being. This acknowledgement includes your guest(s) participation in any tours and evening events.
The Lifesavers Conference plans to take photographs during the 2012 conference and reproduce them in Lifesavers educational, news, or promotional material, whether in print, electronic or other media, including the Lifesavers website. By participating in the Lifesavers 2012 Conference, you grant Lifesavers to reserve the right to use your name and photograph for such purposes. All postings are property of Lifesavers, and may be displayed or used by Lifesavers for any purpose.
